If your project involves any of following, approval is required:

  • Human Subjects

  • Human/Animal Tissue (anything with a backbone)

  • Potential Pathogens (harmful bacteria, viruses, mold)

  • Hazardous Substances, Devices or Activities

Fill this out prior to the start of testing, save it, and attach to the inside of your science fair journal.

If your project involves testing on people of any age, use this form for them to give permission. It can be tailored to fit your project. This must also be presented at the science fair in your journal or binder.
